If you need financial assistance for eye care and receive no form of private or federal insurance, you may be able to qualify for Alabama Lions Sight.
To fill out an application, visit your county Department of Human Resources or you may apply at the Alabama Lions Sight office, 700 18th Street South, Birmingham. You may also contact your local Lions Club Sight Chairman to complete an application. To find out who your local Sight Chairman is, call Alabama Lions Sight at 1-800-967-4448, and dial 1 for Patient Services.
You MUST bring the following in order to complete your application:
Proof of income (2 months of paycheck stubs, Proof of SSI, Proof of Unemployment, etc.) for everyone in the household
Photo ID and Social Security Card
Up-to-date Food Stamp Report (if applicable)
Letter from parties helping you pay your bills (if applicable)
Proof of any kind of financial assistance (Section 8, etc.)
Copies of receipts from medical bills you pay monthly
Copies of all insurance cards
You may also want to bring copies of your utility bills as you will be asked to provide information regarding your monthly expenses.
